Community Healthcare Networks are being implemented across the country to deliver this change.
They will re-shape how we deliver community healthcare services. Implementing these Networks supports our aim of moving to a system that sees more care delivered in the community. These Networks will support people to live more independently in their community by coordinating and integrating services.
